Medication Information for Parents/Guardians:

Allergy Care Plan Form - This form must be submitted annually if your child has an allergy that will require medication at school.  ie. Epipen, Benadryl.

Asthma Care Plan Form  - This form must be submitted annually if your child has asthma and will require medication at school. ie. Albuterol inhaler.

Seizure Care Plan Form - This form must be submitted annually if your child has a seizure disorder.                                                       

Medication Form - This form is required annually for any medications to be taken at school, whether they are over the counter or prescription.                                                 
This form is NOT for allergy, asthma or seizure medication.
