Volunteer forms for the 2023/2024 school year are now available.  Forms may be downloaded and printed using the link below or picked up in packet form from the Brook Knoll office. 

Individuals who are cleared for the 2023/2024 school year may volunteer inside classrooms.  Volunteers must contact their student's teacher for specific details.

All individuals who wish to volunteer on campus in any capacity and/or volunteer to drive on field trips must complete a volunteer packet and have the appropriate documents on file in the school office at least two weeks prior to volunteering. 

Volunteer Packets and instructions can be found in the front office at school, or downloaded and printed from the link below.  Completed packets are to be returned to the front office. 

Driver Registration Forms can be found in the front office at school or downloaded from the link below.  Completed forms are to be returned to the front office with a current copy of your driver's license, vehicle registration, and insurance declarations page.

• Every driver must also complete a Field Trip Notice and Medical Authorization - Adult form located in the front office or by downloading the form from this Link to Field Trip Notice and Medical Authorization - Adult
